Lessons Learned

The only way that the lessons learned from the follow-up after an incident that might prevent another incident is if the lessons learned get translated into lessons “do.”  As Tony Muschara states. “ Learning does not only involve the acquisition of new knowledge.  It also demands a response - a correction of behaviour.”  So without the application of lessons learned, without a change in behaviour and without action, lessons never truly get learned. It is through the application and institutionalization of the knowledge gleaned from previous incidents (regardless of their source) that makes them become truly learned.  Industrial / work settings are filled with examples of repeating the same mistakes over and over again, either their own or those of others. The reason for this replication is the inability of the organization to truly learn the lesson through the disciplined application of an actioned response and the correction of behaviour.  As Muschara implies, without application and a correction of behaviour, lessons are not truly learned.

Anyone can learn from their own mistakes, a wise man learns from the mistakes of others.  Proverb
